Postup pri podávaní žiadosti o primerané úpravy a podporné služby
1. Študent, ktorý sa z dôvodu svojich špecifických potrieb bude pri štúdiu na UK uchádzať o primerané úpravy, podporné služby a/alebo akékoľvek zvýhodnenie, podáva na študijnom oddelení svojej fakulty písomnú žiadosť o zaradenie do evidencie študentov so špecifickými potrebami (formulár žiadosti o zaradenie).
- Študenti 1. ročníka podávajú žiadosť o zaradenie do evidencie spolu s návratkou potvrdzujúcou nástup na štúdium,
- študenti vyšších ročníkov podávajú žiadosť o zaradenie do evidencie študentov so špecifickými potrebami spravidla pri zápise na začiatku akademického roka.
- Žiadosť o primerané úpravy a podporné služby možno podať aj v priebehu akademického roka, priznanie primeraných úprav, podporných služieb a/alebo zvýhodnení však nemá spätnú platnosť.
2. Povinnou prílohou žiadosti o zaradenie do evidencie študentov so špecifickými potrebami je potvrdenie o zdravotnom postihnutí alebo potvrdenie o poruchách učenia.
- Potvrdenie o zdravotnom postihnutí vystavuje odborný lekár, o.i. obsahuje informáciu o vplyve postihnutia na sociálne a pracovné fungovanie študenta. Potvrdenie na začiatku štúdia nie je staršie ako 3 mesiace (odporúčaná štruktúra potvrdenia o zdravotnom postihnutí – pozri formulár).
- Potvrdenie o poruchách učenia vydáva odborné pedagogicko-psychologické pracovisko, štandardne je jeho súčasťou posudok psychológa a špeciálneho pedagóga (odporúčaná štruktúra potvrdenia o poruchách učenia – pozri formulár).
3. Študent je povinný do 5 pracovných dní odo dňa zápisu na štúdium kontaktovať Centrum podpory študentov so špecifickými potrebami alebo fakultného koordinátora pre študentov so špecifickými potrebami za účelom prípravy žiadosti o primerané úpravy a podporné služby (formulár žiadosti o primerané úpravy). Študent prvého ročníka je povinný kontaktovať Centrum podpory alebo fakultného koordinátora ešte pred zápisom.
4. O zaradení do evidencie študentov so špecifickými potrebami a priznaní primeraných úprav a podporných služieb na základe návrhu primeraných úprav a podporných služieb rozhoduje dekan príslušnej fakulty UK.

Application procedure for reasonable adjustments and support services
1. A student who, because of his/her specific needs, will apply for reasonable adjustments, support services and/or any benefits while studying at CU, shall submit a written request to the study department of his/her faculty to be included in the register of students with specific needs (application form for inclusion).
- 1st year students submit an application for registration together with a return slip confirming their enrolment,
- upper year students apply for inclusion in the register of students with special needs, usually at the time of enrolment at the beginning of the academic year. Application for reasonable adjustments and support services may also be made during the academic year, but the granting of reasonable adjustments, support services and/or benefits is not retroactive.
2. A certificate of disability or a certificate of learning disability is a compulsory attachment to the application for inclusion in the register of students with specific needs.
- The disability certificate is issued by a specialist doctor and contains information about the impact of the disability on the student's social and occupational functioning. The certificate shall not be older than 3 months at the beginning of the studies (structure of the disability certificate - form).
- The learning disability certificate is issued by a professional educational-psychological workplace, and normally includes an assessment by a psychologist and a special pedagogue (recommended structure of a learning disability certificate - see the form).
3. The student is required to contact the Faculty Coordinator for Students with Specific Needs within 5 working days of the date of enrolment to prepare a request for reasonable adjustments and support services (reasonable adjustment request form). The first-year student is required to contact the faculty coordinator prior to enrolment.
4. The Dean of the Jessenius Faculty of Medicine at CU in Martin decides within 30 calendar days on the inclusion in the register of students with specific needs and on the granting of reasonable adjustments and support services on the basis of the proposal for reasonable adjustments and support services in cooperation with the committee for the evaluation of the needs of students with specific needs.
The decision to place the student on the register of students with specific needs and to grant reasonable adjustments and support services will be communicated to the student, the Faculty Coordinator, the Support centre for students with SN and the study department of the faculty. The student will present the decision to the course guarantor at the beginning of the semester or from the effective date of the decision.

Legislation
Support for students with specific needs is regulated by the Internal Regulation No. 23/2014 - Directive of the Rector of the Comenius University in Bratislava on ensuring a generally accessible academic environment for students with specific needs. The regulation is valid and effective from 15 October 2014.
